To provide a Community Consultant Based Adult emergency and routine outpatient service.
The aims of the service are to provide a patient focused, high quality ophthalmic service within the local community, meeting 18 week pathway targets, either booked directly or indirectly via choose and book or a faxed referral.
The service is delivered by Consultants/Doctors whom are based in a community setting, providing Continuity of Care within the community.
Patients seen by the ACES Team will expect to be seen within three weeks from referral on average. If the patient requires cataract surgery, the patient then chooses the most suitable date of surgery with the Consultant, with a timescale of a further two weeks on average. Patients requiring minor surgery will expect to wait on average four weeks, making the total waiting time six weeks from referral to treatment. Surgery will be carried out at our surgical locations at either Wisbech or Fakenham.
Reduce waiting times for patients with eye conditions suitable for day surgery.
Give all patients the same choices of providers and location via choose and book service.
Provide Care Closer to Home for the patient in line with DoH guidelines.
Reduce overall waiting times in the health economy.
Create additional capacity in Secondary Care for those conditions excluded from the community service, patients with active diabetic retinopathy needing Argon laser treatment, children, acute diplopia and patients requiring a general anaesthetic.
Aim to provide a fast and efficient patient focused pathway in collaboration with patient representatives and Primary Care Professionals.
Use new technologies and the latest equipment, striving to maintain our care at the cutting edge of ophthalmology.
Ensure that we comply with all regulatory requirements and encompass the latest national and local guidelines.
Ensure that we comply fully with the Disability Discrimination Act 2005, with reference to all staff and patients using the service.
